How to fill out an application for registration of an individual entrepreneur

In order to formalize your business as an individual entrepreneur, an individual must fill out a registration application in the form of p21001. The form of this document was approved by Appendix No. 18 to the Decree of the Government of the Russian Federation No. 439 of 06/19/2002.

You will need

Form application for registration of an individual entrepreneur in the form of p21001, a document on the payment of state duties, documents of an individual, pen, signature and seal of a notary public.

On the first page of the application, indicate the name of the registering authority at your place of residence and its number, enter the surname, first name, middle name, date and place of your birth in accordance with the identification document, Fully write down the address of your permanent place of residence (zip code, region, city, settlement, street name, house number, building, apartment) and contact phone number, fax number (if any).

On the second page of the document, enter the details of the identity document (series, number, date of issue and name of the authority issuing it). In case of opening a company, which is an individual entrepreneur, a person who has not reached the age of majority, indicate the basis for acquiring the citizen's legal capacity from the list provided, write the type and details of the document confirming this right (number, date and name of the authority that issued it). If the enterprise is opened by a foreign citizen, indicate the details of the international document, the type and data of the document giving the person the right to reside in the territory of the Russian Federation.

Indicate on the sheet A of the application the code of the type of economic activity in accordance with the All-Russian Classifier of Types of Economic Activity and the name of the type of economic activity.

Pay the state fee in any bank of the Russian Federation in the amount of four thousand rubles, a receipt confirming the payment made, with the signature of the cashier and the seal of the bank, attach to this application.

On the third page, confirm the accuracy and completeness of the information presented in the document with a personal signature, enter the taxpayer identification number, if available. Verify your signature with a notary, who, in turn, will sign the application in the appropriate field, certify it with a seal and indicate his tax identification number.

Attach the necessary package of documents to the application for registration, depending on your status, submit to the registration authority that will write the receipt in its receipt.
