You need to fill out the application and its annexes on a computer, typewriter or in block letters in black or blue ink.

Make copies of documents confirming the changes (passport, marriage certificate, etc.). Also make a photocopy of the TIN and state registration of the entrepreneur. Sew all documents including more than one sheet and, on the back of the staple, stick paper on the back with the number of sheets, date, your signature and its decryption.
