How to get a psychologist license

The entrepreneurial type of activity that is carried out on the territory of Russia is considered legal only if there is a certain permit document. Such a document is a license. In turn, a license is also required to provide psychological assistance or other medical services.

Collect the necessary set of documents:

- documents that indicate compliance of employees with the necessary professional level for a particular type of activity;

- a document confirming the availability of a working room and compliance with all required sanitary standards. These documents must be certified by a notary;

- certificates for the use of technologies;

- passports for instruments, medical equipment and vehicles (if any);

- constituent, registration documents;

- a decision from the sanitary and epidemiological station;

- Goskomstat codes;

- documents confirming the qualifications of the head of this legal entity (certificate, diploma, work book);

- a document on the right of ownership or rental of premises

- confirmation of the state of technical equipment in the working room, expert opinion.

Write an application for a license. An entity wishing to carry out certain types of medical activity that needs to be licensed independently or with the help of a specific person authorized by it should contact the special licensing bodies with a written application in the prescribed form to issue the necessary license.

Indicate the following information in the application:

- information about the business entity (applicant). Its name, location, bank details, identification code (legal entity). Then write the name and identification number of the individual, and also note his passport data;

- type of business (provision of medical care) for the implementation of which you intend to obtain a license.

Pay the required amount (fee) for considering your application. Save the receipt.

Provide all the documents collected with the application and receipt to the state appropriate structure.
