In accordance with Order No. 376н of the Ministry of Health and Social Development of the Russian Federation dated August 01, 2008, commercial and non-profit organizations annually submit a certificate confirming the main type of economic activity to the executive body of the Fund at the place of registration to confirm the main type of economic activity.

Fill out the table entitled "Distribution of income and revenues for the previous financial year." In it, write in the order of names of types of economic activity, indicate the income for each type of economic activity (in thousands of rubles), as well as target revenues and financing (including budget financing, grants, etc.). Calculate the share of income and income corresponding to the OKVED code for each type of economic activity in the total amount of income and income (in percent). For non-profit organizations, indicate the number of employees for the previous year for each type of economic activity.

Calculate the total amount of income, earmarked income and financing for commercial organizations, for non-profit - the total number of employees.
