How to apply for a re-issuance of a certificate?

An entrepreneur should apply for the re-issuance of a USRIP certificate to the tax inspectorate, where he is registered as the subject of the relevant activity. When applying, a handwritten and signed application is provided in which the entrepreneur indicates the need to re-issue a certificate of state registration. The application shall be accompanied by a receipt, payment order or other documents confirming the introduction of a state fee into the budget. The re-issuance of a certificate, like the state registration of an individual entrepreneur, is considered a state service, which is provided only when such a fee is paid.
